Live Well. Die Well. is a year-round community education curriculum grounded in our core values of compassion, dignity and presence. Open to anyone seeking a deeper understanding of end-of-life care, this series explores the essential topics below and offers an opportunity for intergenerational exchange. Designed for both caregivers and curious learners, each session offers meaningful guidance, practical insight and a welcoming space to learn, reflect and grow.

Planning Ahead (nursing CEs optional)

Learn how intentional planning can reduce stress, support loved ones and ensure that your wishes are honored. This session introduces practical tools and conversations that help you prepare for healthcare decisions, personal preferences and the unexpected.

What Is a Social-Model Hospice Home?

February

Discover the heart and purpose of Social-Model Hospice Homes - community-supported spaces that offer comfort, safety and compassion at the end of life. This session explores what makes these homes unique, how they partner with hospice agencies and families and why they are a vital part of the care ecosystem.

Compassionate Honesty

March

Explore what it means to communicate truthfully and tenderly during times of vulnerability. This session discusses how honest conversations can build trust, ease fear and deepen connection when navigating serious illness and end-of-life experiences.

Advance Care Planning (THDD)

April

Understand the documents, decisions and conversations that make up advance care planning, including the role of the Oklahoma Advance Directive for Healthcare form. Participants will learn how to clarify personal values, communicate choices and complete the necessary steps to ensure their wishes are respected.

Caring For (nursing CEs optional)

This session focuses on the emotional, relational and practical dimensions of caring for someone facing the end of life. Participants will learn approaches that honor dignity, meet changing needs and create meaningful moments of connection.

Physical Care of Dying People

June

Learn foundational skills for providing gentle, safe and effective physical care to someone at the end of life. This session covers comfort measures, positioning, nutrition and hydration considerations, and how to respond to common changes in the body.

Psycho-Social Care

July

This session delves into the psychological and social needs of people nearing the end of life, as well as the needs of their families. Participants will learn how to support emotional expression, reduce isolation and create environments of comfort and connection.

Being With (nursing CEs optional)

August

Explore the power of presence - being fully with someone without needing to fix or change their experience. This session offers guidance on deep listening, quiet companionship and holding space for another person with compassion.

Death and Dying 101

September

Gain a clear, compassionate overview of what to expect at the end of life. This session covers common physical, emotional and spiritual changes, helping demystify the dying process for caregivers and community members alike.

Mortality Salience

October

Consider how awareness of our own mortality shapes the way we live, love and make meaning. This session invites reflection on fear, avoidance, and acceptance—and how acknowledging impermanence can enrich daily life.

Tending After (nursing CEs optional)

November

Learn what happens after a death - both practically and emotionally - from immediate next steps to the rituals and actions that help families navigate early grief. This session offers guidance in supporting others and caring for oneself during this tender time.

Grief, Mourning, Grieving & Bereavement

December

Explore the wide range of experiences that accompany loss, including the differences between grief, mourning, grieving and bereavement. This session provides compassionate insight into how individuals process loss, what supports healing and how communities can companion one another.
