Recognizing Excellence in End-of-Life Care

2009 Award Categories:

End-of-Life Advocate Award - Nominee will demonstrate visionary leadership in promoting & advocating for access to end-of-life care in the community. Some examples may be a social worker, chaplain, hospital case manager, or philanthropist.

Clinical Excellence Award - Nominee will demonstrate exemplary skills in hands-on patient care, symptom management, interdisciplinary teamwork, and creative approaches to patient and family centered care. Some examples may be a nurse, private sitter, massage therapist or home care aide.

Volunteer Excellence Award - Nominees must currently volunteer with an organization focused on end-of-life care, exemplify the hospice concept; demonstrate compassion, commitment to service and advocacy for quality choices at life’s end.
